This document provides information about the continuation of benefits for employees while they are on leave under the Family and Medical Leave Act (FMLA).
Under FMLA, Research Foundation (RF) health, dental and vision insurance benefits in effect at the time of the leave will be continued up to 12 weeks for an employee on approved FMLA leave. This period can be extended to 26 weeks for employees caring for a military family member who is seriously injured in the line of duty. Life insurance may be continued at the employee's option.
Employees who choose to continue their benefits must complete an application and attach a copy of the approved employee request for leave.
Employees on FMLA leave are charged the biweekly employee premium for health and dental insurance for each pay period missed. Vision is continued without charge to the employee.
Employees on FMLA leave can continue basic life insurance by paying the full Foundation premium. Employees may also continue optional life insurance by paying the employee premium.

Premiums should be paid in advance while on leave. Full payment can be deducted from the last paycheck if payment of the health premium on a pre-tax basis is desired, or can be paid monthly or biweekly.
If payment is not received within 30 days of the due date, coverage will be cancelled.
Upon return from FMLA leave, benefits will be reinstated without a waiting period whether or not they are continued during this leave.
If an employee does not return to work after FMLA leave expires, the full share of the premium paid by the Foundation can be recovered. An exception is made if the employee does not return because of circumstances beyond the control of the employee.
An employee continuing on Leave Without Pay (LWOP) under Research Foundation leave policy (a combined maximum of one year), should contact the office at the operating location that handles employee benefits for information on premium payment under LWOP.
