An employee working at least 50 percent of full time on a regular appointment and whose annual salary is at least $15,000 is eligible to enroll in the plan.
If you enroll for the plan when you are first eligible to do so, you are guaranteed coverage without medical examinations or questions, and you will not be subject to any pre-existing condition exclusions.

You may enroll via Employee Self Service (www.rfsuny.org/selfservice) during your initial 60 days of eligible employment. Enrollment at any other time must be done by submitting a completed Benefits Enrollment form and Voluntary Short-Term Disability Enrollment Form to your campus Benefits Office.

You can enroll or increase coverage up to the plan limits effective January 1, 2016 without providing evidence of insurability. Any claims within the first 12 months of any new or increased coverage will be subject to a pre-existing condition exclusion. So if you were diagnosed in the last three months of 2015 with a condition for which you file a claim in 2016, all or part of your claim may be denied. For example if you are pregnant now, your pregnancy disability claim will not be paid for any new amounts you elected during this special enrollment period.
